– The Holmes County General Health District is hosting a drive-thru COVID-19 Moderna booster vaccine clinic. The clinic will be held at the Holmes County Fairgrounds on Tuesday November 16, 2021 from 9am to 1pm. You will enter the clinic on Township Road 301 and exit the clinic through the church parking lot. To receive the Moderna booster vaccine it is required to have received both doses of the Moderna COVID-19 vaccine and to have received the second dose at least six months ago. Moderna booster vaccines will only be given to those who are eligible and have an appointment. You do not need a doctor’s note to the clinic. Please bring the following items with you to the booster clinic: your COVID-19 vaccination card, your completed registration form, a mask, and your ID. You can download the registration form at
holmeshealth.org and complete this form prior to arriving to the clinic to shorten wait times. If you have already called into the health department and put your name on a waiting list, a representative will call you to make an appointment. If your name is not on our waiting list, to schedule an appointment you may call the Health District at 330-674-5035 M-F 8am-4:30pm
